NOTICE TEXT: Department of the Army U.S. Army Medical Command Western Regional Contracting Office This a Combined Synopsis/Solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation For Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only synopsis/solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation# W91YU0-09-T-0087 is issued as a request for quotations (RFQ). The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36 and DAC 20090825. The associated North American Industrial Classification System (NAICS) code for this procurement is 423410. The Size Standard for small business is 100 employees. This RFQ is being issued as a 100% Small Business Set-Aside. This requierment is for Madigan Army Medical Center, Tacoma, WA 98431. A site visit is scheduled for Monday, September 21 at 10:00. Interested parties must contact Ms. Brindle Summers at brindle.summers@us.army.mil or at (253) 968-4016 prior to the site visit. No additional site visits will be scheduled. Vendors should meet outside of the entrance to the Madigan Caf, located at Old Madigan on Fort Lewis, BLDG 9903. The street address is 9903 Lincoln Street, Tacoma, WA 98431. If you do not have a pass to get on base, you must first get a visitors badge by reporting to the Visitors Center outside of Fort Lewis main gate (Exit 120). To be issued a pass, you must provide a current ID and valid car registration and insurance. Please allow at least one hour to get your badge at the Visitors Center. Questions regarding this announcement shall be submitted via email to Ms. Brindle Summers at brindle.summers@us.army.mil no later than 12:00 Noon on Wednesday, September 23, 2009. In accordance with the attached Instructions to Offerors (Addenda to FAR 52.212-1), all responsible Contractors shall provide a quote to provide all labor, equipment, material, software/equipment licensing, supervision, and warranty necessary to install a turn-key, wired, Closed Circuit Television Medical Evaluation Training System, complete with video cameras, video mixing devices, microphones, video processing and grading station, mobile computers, network storage devices, network routing devices, and device housings. All equipment must be compatible with government-owned Mac computers. Upon completion of installation, the contractor shall provide training to government personnel on use of the system and warranty all equipment for a period of five years. Interested vendors shall review all attachments to this combined synopsis/solicitation and respond accordingly.
